Liberty ARC is Hiring an Admissions Coordinator Near Amsterdam, NY

Admissions CoordinatorLocation: Montgomery County, NYFLSA: Non-exemptReports To: Director of Residential & Day ServicesPhysical requirements:  Normal Office Environment Hours of Work: 8:00 – 4:00 Monday – Friday 37.5 hours per week with some evening hours required.Pay Range: The pay range is $18.40 to $26.28Job Summary: The Admissions Coordinator will oversee all tasks related to the placement of individuals in all Liberty programs. This will include, but not be limited to, screening all referrals, chairing the future options committee, facilitating all pre-admission and admission meetings for day and residential programs, completing all admission paperwork for the agency and OPWDD, being the liaison between Liberty and other community agencies, schools and families, obtaining initial eligibility for individuals and aligning them with services.

Job Qualifications:

  1. Bachelor’s Degree or a combination of education and specialized experience which in the judgement of the agency is substantially equivalent to the standards of this subdivision.
  2. Presentation skills and good communication skills are required.
 

Major Responsibilities:

  1. Represent the agency at the Regional OPWDD meetings and accept appropriate referrals.
  2. Act as Liaison for Montgomery County Special Education which includes keeping them informed of Liberty’s Programs, meeting with CSE’S, attending meetings as requested and transitioning students to Liberty programs.
  3. Serve as the primary point of contact with individuals, families and Care Coordinators as referrals are being assessed.
  4. Chair the future options committee and ensure all follow up is completed.
  5. Maintain familiarity with the IRA’S and day program sites and ensure there is good communication with the Director’s, Associate Directors, Managers, Care Coordinators, Clinical Supervisors and staff.
  6. Meet with families to discuss Liberty services and do an initial screening.
  7. Complete all required OPWDD documentation for vacancy management and ensure it is submitted to the state in a timely manner ie: vacancy reports, internal move approval forms, screening reports, home description forms.
  8. Assist in the development and implementation of policy and procedure around admissions for FSS, Day, and Residential programs.
  9. Coordinate all emergency respite stays ensuring informational meetings are taking place with the team members.
  10. Act as the liaison between Facilities and operations during all Residential moves
